General information

  • - Provides the creation, improvement, development, support, support of the functioning of the Register of the territorial community of Kharkov, and also carries out its formation and maintenance
  • - Carries out registration/removal from the registration of the place of residence/stay of persons within the city of Kharkov
  • - Ensures the transfer of information and/or the introduction, in the manner prescribed by law, of information on registration/removal from the registration of the place of residence/stay of individuals in the Unified State Demographic Register
  • - Carries out coordination and methodical management of activity of its structural divisions
  • - Carries out, in accordance with the current legislation, the control over observance of the legislation by its structural divisions in the field of registration/removal of registration of the place of residence of individuals
  • - Gathers, analyzes and reduces the statistical information in the field of registration/removal of registration of the place of residence of individuals
  • - He considers complaints about the decisions of officials of his structural divisions and initiates the issue of the abolition of the said decisions, if they were taken in violation of the current legislation
  • - Acceptance of citizens on matters within the competence of the Office, considers their appeal
  • - Provides and organizes the proper preparation of responses to inquiries and letters from executive authorities, local governments, enterprises, institutions and organizations on matters that fall within the competence of the Office
  • - Ensures the proper storage of seals and stamps obtained by the Office employees for the performance of assigned tasks in the field of official activity
  • - Ensures observance of labor discipline, rules of internal labor regulations and requirements of labor legislation
  • - Provides proper record keeping, document management of the Office
  • - Provides compliance with the management of the proper level of executive discipline and the timing of specific tasks
  • - Studying the positive experience in the area that falls within the competence of the Office, generalizes the practice of applying legislation on matters that fall within the competence of the Office, prepares proposals for its improvement and submits them for consideration to the Director of the Department
  • - Prepares draft decisions of the Kharkiv City Council orders the mayor of Kharkiv on within the competence of management
  • - Prepares materials for their placement in the mass media on the state of formation and maintenance of the Register of the territorial community of Kharkiv, registration/removal of registration of the place of residence/stay of persons, the transfer of information and/or entering in the manner prescribed by law information on registration/removal from registration of the place of residence/stay in the Uniform State Demographic Register
  • - Informs the Director of the Department of the status of implementation by the Office of the tasks assigned to the Office
  • - Performs the decision of the sessions of the Kharkiv City Council, the instructions of the Kharkiv mayor, orders and orders of the Director of the Department
  • - Carries out other functions aimed at realization of the purpose and direct execution of the tasks entrusted to the Office, execution of decisions of the sessions of the Kharkiv city council, instructions of the mayor, orders and instructions of deputy mayors, instructions of the director of the department, etc

Registration, removal from registration of place of residence, stay

To register a place of residence / stay the person or his representative submits:

  • - Application for the forms given in Annexes 6, 7 or 8 respectively to the Rules of Residence:
    • - Application for registration of the place of residence (Annex 6)
    • - Application for registration of a minor child's place of residence (Annex 7)
    • - Application for registration of place of residence (Annex 8)
  • - The document to which the information about the place of residence / stay is submitted (passport of a citizen of Ukraine, temporary certificate of a citizen of Ukraine, permanent residence card, temporary residence permit, refugee certificate, identity card requiring additional protection, identity card for temporary protection, certificate about the appeal for protection in Ukraine)
  • - Birth certificate (for persons under 16 years of age)
  • - A receipt for payment of an administrative fee (in case of registration of the place of residence simultaneously with the removal from the previous residence administrative fee is charged for only one service)
  • - Documents confirming:
    • - The right to reside in a home - a warrant, a certificate of title, a contract of employment (sublease, lease), a court decision which has come into force, the granting of a right to the person to enter the living space, recognition of the right of the person to use the apartment or ownership of him, the right to register a place of residence or other documents
    • - In the absence of these documents, the registration of a person is carried out with the consent of the owner / co-owners of the dwelling, the employer and his family members for registration of the place of residence (these documents or consent are not required when registering the place of residence of minors at the address of registration of the place of residence of parents / one of the parents or legal representative / representatives)
    • - The right to stay or to register at a specialized social institution, institution of social service and social protection of a person - a certificate of acceptance for service in a specialized social institution, a social service institution and a social protection of a person in the form, in accordance with Annex 9, a copy of the certificate of taking on the account of a homeless person, the form of which is approved by the Ministry of Social Policy (for persons who are registered in these institutions or institutions)
  • - Military ticket or residence permit (for citizens who are subject to military registration or are in military registration)
  • - Application for removal of the person from the registration of the place of residence in the form according to the Appendix 11 to the Rules of registration of the place of residence (in case of registration of the place of residence with the simultaneous withdrawal of registration of the previous residence). In case of submission of an application, the representative of the person is additionally filed:
    • - Identity document
    • - A document confirming the authority of a person as a representative, except when the application is submitted by legal representatives of a minor child - parents (adopters)
  • - Registration of the place of residence of a person on the application of a legal representative is carried out with the consent of other legal representatives
  • - In the case of registration of the parents' place of residence at different addresses, the place of residence of the child under 14 years old shall be registered with one of the parents, with the written consent of the other parent in the presence of the person who accepts the application, or on the basis of a written confirmation of the second parents (except for cases where the place of residence of the child is determined by the relevant court decision or decision of the guardianship and trusteeship body)
  • - Registration of the place of residence of the persons who applied for protection in Ukraine with the application form in accordance with Appendix 8 (application for registration of the place of stay), shall be carried out on the basis determined by the law on the basis of the above list of documents, except for the receipt on payment of the administrative fee

The person or his representative submits for the removal from the registration of the place of residence:

  • - Application form according to Annex 11 to the Rules of registration of the place of residence, approved by the Resolution of the Cabinet of Ministers of Ukraine dated 02.03.2016 No. 207 (hereinafter - the Rules) (application for removal from registration of a place of residence)
  • - The court decision, which came into force, concerning the deprivation of the right to own a living quarters or the right to use the living quarters, eviction, the removal from registration of the residence of the person, the recognition of a person missing or declaring her dead
  • - Death certificate
  • - Notification of the territorial office or department of the LCA with the indication of the relevant details of the passport of the deceased person or death document issued by the competent authority of the foreign state legalized in the established manner
  • - Other documents that indicate termination:
    • - Grounds for staying in the territory of Ukraine of foreigners and stateless persons (information of the territorial body of the LCA or territorial division of the LCA, in whose territory the service is registered, the residence of the person, the expiry of the validity of the temporary residence permit, or a copy of the decision to cancel the temporary residence permit or revoke the permit for immigration and residence permit in Ukraine)
    • - Grounds for accommodation of a homeless person in a specialized social institution, a social service and social security institution (a written statement from a social institution, a social service and social security institution)
    • - Grounds for the right to use the living space (expiration of the contract of lease, hiring, sublease of living space, period of study at an educational institution (in the case of registration of the place of residence in the dormitory of the educational institution at the time of studying), alienation of housing and other documents determined by the legislation)

Withdrawal of registration of a place of residence in connection with the termination of grounds for the homeless person's stay in a specialized social institution, a social service and social security institution, and termination of the grounds for the right to use residential premises is carried out at the request of the authorized person of the institution / institution or at the request of the owner / tenant of the dwelling or their representatives.

Withdrawal of registration of the place of residence of orphan children and children deprived of parental care, persons for whom a guardianship or trusteeship is established, shall be carried out in consultation with the authorities of guardianship and guardianship.

Together with the statement the person submits:

  • - A document to which information on the removal of registration of residence is made. If the child is under the age of 16, a birth certificate is issued
  • - Receipt for payment of administrative fee
  • - Military ticket or residence permit (for citizens who are subject to military registration or are in military registration)

If the application is filed by a representative of a person, in addition to the indicated documents, additionally filed:

  • - Identity document
  • - A document confirming the authority of a person as a representative, except for cases when the application is submitted by legal representatives of a minor child - parents (adopters)

Withdrawal from registration of the place of residence of a person on the application of a legal representative is carried out with the consent of other legal representatives.

In the case of registration of the parents' place of residence at different addresses of the registration of the place of residence of the child under the age of 14, together with one of the parents, the written consent of the other parent in the presence of the person accepting the application or on the basis of a written the consent of the second parent (except for cases where the place of residence of the child is determined by the relevant decision of the court or the decision of the guardianship and trusteeship body).

Registration of a newborn child's place of residence through state registration bodies of civil status acts

An exhaustive list of documents necessary for registration of a newborn child's place of residence through state registration bodies of civil status acts:

  • - Application for registration of a minor child's place of residence (Annex 7 to the Rules of registration of the place of residence, approved by the Resolution of the Cabinet of Ministers of Ukraine dated 02.03.2016 No. 207)
  • - Birth Certificate
  • - Receipt for payment of administrative fee
  • - A document certifying the identity of the legal representative

Registration of the place of residence of a person on the application of a legal representative is carried out with the consent of other legal representatives.

In the case of registration of the parents' place of residence at different addresses, the place of residence of the child under 14 years old shall be registered with one of the parents, with the written consent of the other parent in the presence of the person who accepts the application, or on the basis of a written confirmation of the second parents (except for cases where the place of residence of the child is determined by the relevant court decision or decision of the guardianship and trusteeship body).
